Immunizations
Immunizations are required for students to register and attend school in Arizona (A.R.S 15-871 & 15-874). Below is a link to the Arizona School Immunization Requirements for Kindergarten through 12th grade. Please review this form and make sure your child has the appropriate immunizations for his/her age and grade.
I will review all records and if your child is missing any immunizations required for school, I will contact you and let you know what is needed. As a reminder, there are immunizations required for students who are 11 years old. You will be allowed a grace period after receiving a notice of missing immunizations to get the necessary shots. After the grace period, if you have not gotten your child's immunizaitons up to date, s/he will not be allowed to attend school until the required shots are received. After completing the required immunizations, you must provide me with a copy of the proof of immunziations. You can scan and email them, mail them, or drop them off in the office.
FREE IMMUNIZATIONS ARE AVAILABLE! (for school required immunizations). If you choose to utilitze this clinic, bring your immunization record and insurance card.
